. Position Duties:
I. Review records and compile reports for use by RNs/LPNs providing care
management to the MaineCare Member population;
II. Provide reports and monitor MaineCare Member outcomes in accordance
with established procedures;
III. Communicate with all medical care personnel providers, employers, health
insurance companies and MaineCare Members in order to determine
Member level of care and appropriate community support services available
to Members;
IV. Coordinate MaineCare Members with appropriate community resources and
act as co-lead on care management materials sent to Members including
quarterly newsletters, letters, brochures and palm cards;
V. Provide support to RNs/LPNs by attending meetings, taking notes and
compiling action items; and
VI. Perform other duties and responsibilities as may be assigned by the
Department.
Requirements:
I. A two (2) year combination of education and experience in: care
management; interviewing and data gathering skills necessary to identify
available community resources; and MaineCare Member eligibility
determination.
