Position Duties:

I. Provide case management, support and community integration services that involve the identification, assessment, planning, linking, monitoring and evaluation of services and supports needed by MaineCare Members;

II. Collaborate with the RNs/LPNs and LCSW and monitor the individualized support plans prepared by the Community Integration Worker working for the community agency providing direct services to MaineCare Members;

III. Work with other state agencies for efficacy in conjunction with MaineCare Members and collaboratively update and revise the individualized support plan as needed.

IV. Provide information, consultations and problem-solving assistance and support to MaineCare Members and the family or members of the MaineCare Member's support system, including providers;

V. Facilitate and coordinate team meetings as appropriate; and

VI. Perform other duties and responsibilities as may be assigned by the Department.

Requirements:

I. A six (6) year combination of education, training, and/or experience providing case management services and;

II. A Mental Health Rehabilitation Technician/Community Certification (MHRT/C) certification.
