Position Highlights:
The Case Manager provides case management services to persons living with HIV/AIDS in a six county area. The position requires ability to function as part of a multidisciplinary, comprehensive HIV service team providing case management services in clients' homes, environments or in the office.

Responsibilities:
Participates in regular clinical supervision, program meetings and peer consultation.
Conducts outreach activities to expand community awareness of case management programs and services.
Completes an assessment of clients' physical, psycho-social, medical, legal and financial wants and needs, and collaborates with clients to develop services plan.
Schedules client contacts depending on client needs or expressed preferences, service plan, program policy and /or case load.
Assists the patient to identify and/or develop their support system and encourages the patient to utilize their support system.
Identifies and effectively utilizes community resources to meet the needs of patients/families.
Collaborates with other healthcare providers and health system administrators in order to optimize outcomes.
Works with clients to assist them in adhering to medication regimens.

The ideal candidate will have:
Licensed Social Worker (LSW) preferred
Bachelor's degree preferred
1 year or more experience preferred
