Position Highlights:
Are you looking for a flexible schedule? Does it excite you to have an opportunity to provide counseling in unique/different settings? In this position, the LCSW will have the opportunity to work in a family practice as well as see patients in a collaborative care center, providing counseling to patients receiving alternative care for pain management.

Responsibilities:
Provides assessment, planning, counseling, and education services for patients.
Implements assigned patient treatment plan and completes all required documentation.
Systematically and continually assesses the patient's mental status in collaboration with the patient/family/providers.
Assesses patient and families needs’ and provides evidence-based outpatient treatment.
Provides group, individual and family counseling.
Identifies and refers patients to appropriate community resources for follow-up care.

The ideal candidate will have:
Maine state licensure as Licensed Clinical Social Worker, required.
