HealthReach Community Health Centers seeks a full-time Outreach/Enrollment Specialist to link patients to health insurance, social services and community education opportunities. Working within a system of 11 community health centers which offer primary care with integrated behavioral health, the selected candidate will assist patients in self-empowerment in the use of health systems towards improved wellness and quality of life. The specialist will be based in Bethel and Livermore Falls and possess excellent organizational, computer and communication skills.
