Summary
The Patient Navigator is responsible for providing health education targeting patients' needs and health risk factors, assisting patients to establish health goals, monitoring patient health outcomes, and serving as a key referral source for patients diagnosed with cancer or identified at high risk for cancer. The Patient Navigator is responsible for helping patients and their families navigate the healthcare system and community-based services by eliminating barriers to care. The Patient Navigator will also engage community partners to conduct preventive health screenings and health risk assessments in the community setting, linking patients without a source of primary care to a Patient Centered Medical Home and/or community-based services. This is a grant funded position. The Patient Navigator will ensure work aligns with and meets all grant guidelines and deliverables.
Education and Experience
High School Diploma/GED or Associates Degree Preferred.
Experience in community engagement, patient advocacy, health education, social work, and/or counseling are strongly preferred. Experience working within a healthcare setting required. Demonstrated success in building effective trusting partnerships required
Driver's license in good standing

Physical Requirements
Intermittent sitting and standing
Sitting at desk; communicating using telephone
Off-site travel throughout service area
